CSM serves individuals with disabilities and our senior citizen neighbors that will bring “Opportunities Within Reach” for our consumers. This service is in addition to our recently initiated Title XIX Waiver Program.
The Aged and Disabled Waiver is a community based model that will serve individuals and their families in their natural living environments, to avoid placement in nursing facilities and enhance self-sufficiency. CSM respectfully requested approval of the application in order to provide choices for consumers and their care providers through a human services agency with an established system of intervention and knowledge of our regional needs. This is the mission of our agency and our newest program will help our fellow citizens achieve the dignity of meaningful life experiences through self actualization and equal opportunities.
The Aged and Disabled Waiver Program provides in-home and community services to individuals 18 years of age and older who are medically and financially eligible. Medical eligibility is determined based on an assessment by a medical professional. Financial eligibility is based on monthly income that cannot exceed 300% of the poverty (SSI) level and assets that cannot be valued at more than $2,000; financial eligibility determination is made at the county WV Department of Health and Human Resources office. Services are provided by Medicaid-enrolled providers throughout the state who follow policy based on the ADW Manual issued by the WV Department of Health and Human Resources. These agencies are monitored for quality assurance by the Bureau of Senior Services’ staff. The program is provided guidance and feedback by a Quality Assurance & Improvement Advisory Council.
The West Virginia Medicaid program offers a comprehensive scope of medically necessary services to diagnose and treat eligible members. Covered and authorized services must be rendered by an enrolled provider within the scope of their license and in accordance with all state and federal requirements. The Aged and Disabled Waiver Program is defined as a long-term care alternative that provides services that enable an individual to remain or return home rather than receiving nursing facility care. Members have a Plan of Care that encompasses Homemaker Services, Case Management, Consumer-Directed Case Management, Medical Adult Day Care, Transportation, and R.N. Assessment and Review.
CSM provides Homemaker Services and Case Management. Eligible agencies are allowed to serve up to eight counties and CSM has been accepted to serve the following: Logan, McDowell, Mercer, Mingo, Monroe, Raleigh, Summers and Wyoming. With the opening, but not yet approved, office in Bluefield, WV, an additional 8 counties can be added as long as they are contiguous. These additional counties are: Greenbrier, Nicholas, Webster, Fayette, Kanawha, Boone, Lincoln, and Wayne.
